There shouldn't be any configuration necessary. Check that you've done these things:

Un-RARed the ISO and put it in the root\ISO directory on your PSP's memory stick.

Have a real UMD in your PSP when you go to run the game. (May not actually be necessary here, but it helped with earlier firmware workarounds.)

Make sure your ISO is good. There may be multiple versions floating around out there. If you know how, check the info hash.

Have you gotten other games to work with your DA3.40OE-A install? It's been a while, so I can't really remember if it's possible to mess that up or not. But if you want to be really thorough you could always restore your firmware, wipe your memory stick, and start from a clean slate. You might want to DL a different ISO too -- there are some that claim to be EUR region (which is impossible -- game wasn't released there), and some that claim to be USA region. If one doesn't work, try the other. And make sure you read the comments for the torrent. If it's bad, people will probably have complained already.

Hope that helps. If you've gotten this far I'm sure you already know a lot of this... but just in case.

Cheers

I tried Dungeon Maker with Dark Alex 3.40 OE-A and can't run this game. I've also tried the option No UMD: OFF, but same thing. How do you configure it to run this game?
